Patient Survey:  Action by 30th March 2018 please

Through our newly formed Patient Participation Group (PPG), we wish to survey our patients and obtain your impression of the VSG to see what works well and what could be improved within the surgeries.

We have designed a simple survey which you can complete on-line and should not take more than 10 minutes of your time. Please feel free to share this link with other members of your family or friends so they can complete this also – the more feedback we get the better!

There will also be paper copies available in both surgery waiting rooms. Once you have completed the survey, this can then be placed into a collection box in the waiting room at either Farndon or Tattenhall Surgery.
