We understand that not everything can be fixed by medicine and many factors influence a person’s sense of wellbeing. It’s key that patients find the appropriate support to help them regain control and take responsibility for their own health.
The Wellbeing Coordinator works with patients who need some guidance and support in a whole range of areas such as support services, Adult Social Care, diet and exercise, emotional health, loneliness, bereavement, financial issues, anxiety and mental health problems and lots more.
They offer face to face appointments in the surgery or telephone consultations to discuss your situation further, make an action plan together to help you move forwards and may also involve some ongoing support sessions.
To access the service, you must be over 18 years and have one long term health condition. To be referred to our Wellbeing Coordinator, please contact the surgery.
